TeleCARE

A Healthcare Access Program

The TeleCare program was created by team of healthcare providers in partnership with STAND! For Families Free of Violence and the Family Justice Center. Telecare aims to provide virtual medical appointments to anyone regardless of insurance and at no cost.

Services Provided

With TeleCare you are connected to wraparound services and free healthcare.

TeleCare Provides:

  • Post-trauma care
  • Management of acute and chronic medical conditions
  • Psychiatry level mental health care
  • Referrals to specialty and long-term primary care
  • Our capabilities include telehealth services, in-person referrals, laboratory testing, referrals for imaging, free physical therapy, free medication coverage and delivery.

Family Justice Center Provides:

  • Access to legal assistance (for example: restraining order, child custody)
  • Housing Assistance
  • Financial and Employment Assistance
  • Wellness
  • Family Support
  • Additional connections to over 60 partners

We follow our clients closely to ensure successful navigation of social barriers.

The Following Is For Professional Use Only

We care for clients who are residing in Contra Costa County and are survivors of interpersonal violence (domestic violence, intimate partner violence, sexual assault, child abuse, elder abuse, and human trafficking) of any form. Client’s experiencing long-term sequela from interpersonal violence qualify. There are no barriers to our services.

Clients can expect to be connected to a wide network of support to help alleviate or resolve their situation.

E-Referral Process

This online form allows for HIPAA compliant referrals to the Family Justice Center for both access to free health care and wraparound services. Referrals can be placed by: physicians, advanced practitioners, mental health specialists, social workers, paramedics, teachers, administrators, attorneys, & counselors.

  2. Fill out corresponding form
  3. A navigator will reach out to the client and assess for any additional services needed to support them.
  4. Referrals will be made for TeleCare & other relevant connections
  5. Continued support and follow-ups
